Job Summary

A company is looking for a Remote Licensed Practical Nurse (LPN) with California Licensure.

Key Responsibilities

• Manage patient census to close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources for patient care
• Provide patient education on chronic condition management and perform medication management

Required Qualifications, Training, and Education

• Active California LPN or LVN license
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Experience with chronic care management
• Ability to work in a remote, HIPAA-compliant workspace
