Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with California licensure for a remote, full-time position.

Key Responsibilities

• Manage patient census to address gaps in clinical and non-clinical care
• Identify and coordinate community resources for patient care
• Provide education on chronic condition management and perform medication management

Required Qualifications

• Active California nursing license (LPN)
• Minimum of 3 years of direct patient-facing experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote, HIPAA-compliant environment
• Experience with Epic and bilingual skills are a plus
