Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with California licensure for a remote position on a day shift.

Key Responsibilities

• Manage patient census with a focus on closing gaps in patient care
• Identify and coordinate community resources to enhance patient care
• Provide patient education on managing chronic conditions and perform medication management

Required Qualifications

• Active California nursing license (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote, HIPAA-compliant environment
• Experience with Epic and bilingual skills are a plus
