California Licensed Practical Nurse

Job Summary

A company is looking for a Remote Licensed Practical Nurse (LPN) with California Licensure for day shifts.

Key Responsibilities:

• Manage patient census and address gaps in clinical and non-clinical care
• Identify and coordinate community resources for patient care
• Provide education on chronic condition management and perform medication management

Required Qualifications:

• Active California nursing license (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote, HIPAA-compliant environment
• Experience with Epic and bilingual skills are a plus
