Job Summary

A company is looking for a Licensed Practical Nurse with a California License required.

Key Responsibilities

• Support healthcare delivery by performing activities and procedures under the direction of Nurse Practitioners
• Provide one-on-one coaching and support to patients managing chronic conditions
• Conduct comprehensive assessments and monitor patients' progress and adherence to treatment plans

Required Qualifications, Training, and Education

• Active multi-state compact Practical Nurse License
• Active California State LPN/LVN license is required
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for individuals managing chronic conditions
• Prior experience working in a high-growth, fast-paced startup environment is a plus
