Job Summary

A company is looking for a Licensed Practical Nurse to provide remote health coaching and patient care.

Key Responsibilities

• Support healthcare delivery by performing activities under the direction of Nurse Practitioners
• Provide coaching to patients managing chronic conditions and help them execute personalized care plans
• Conduct assessments and monitor patients' progress through regular check-ins and education on disease management

Required Qualifications

• Active multi-state compact Practical Nurse License
• Active California State LVN license
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic condition management
• Experience supporting patients remotely is a plus
