Key Responsibilities

• Support healthcare delivery by performing activities prescribed by the Nurse Practitioner
• Provide coaching and support to patients managing chronic conditions
• Conduct assessments and monitor patients' progress and adherence to treatment plans

Required Qualifications, Training, and Education

• Active multi-state compact Practical Nurse License
• Active California State LPN/LVN license required
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic condition management
• Prior experience supporting patients remotely is a plus
