Job Summary

A company is looking for a Licensed Practical Nurse - CA License Required.

Key Responsibilities

• Support delivery of health care by performing activities and procedures under the direction of Nurse Practitioners
• Provide one-on-one coaching to patients managing chronic conditions and help execute personalized care plans
• Conduct assessments of patients' health status and monitor their progress through regular check-ins

Required Qualifications, Training, and Education

• Active multi-state compact Practical Nurse License
• Active California State LVN license
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic conditions management
• Experience supporting patients remotely is a plus
