Job Summary

A company is looking for a Licensed Practical Nurse with a CA License required.

Key Responsibilities

• Support healthcare delivery by performing activities under the direction of Nurse Practitioners
• Provide coaching and support to patients managing chronic conditions
• Conduct assessments and monitor patient progress through regular check-ins

Required Qualifications

• Active multi-state compact Practical Nurse License
• Active California State LVN license required
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic condition management
• Prior experience in a remote patient support role is a plus
