California Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (CA license required).

Key Responsibilities

• Support delivery of healthcare by performing activities and procedures under the direction of Nurse Practitioners
• Provide one-on-one coaching to patients managing chronic conditions and help execute personalized care plans
• Conduct comprehensive assessments and monitor patients' progress through regular check-ins and remote tools

Required Qualifications, Training, and Education

• Active multi-state compact Practical Nurse License and California State LVN license
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic condition management
• Prior experience in a high-growth, fast-paced startup environment is preferred
• Experience in supporting patients remotely is a plus
