Job Summary

A company is looking for a Licensed Practical Nurse (CA license required).

Key Responsibilities

• Support delivery of healthcare by performing activities under the direction of Nurse Practitioners
• Provide one-on-one coaching for patients managing chronic conditions
• Conduct comprehensive assessments and monitor patients' progress through regular check-ins

Required Qualifications

• Active multi-state compact Practical Nurse License
• Active California State LPN/LVN license
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for chronic condition management
• Experience in supporting patients remotely is a plus
