California Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (CA license required) to provide virtual patient support across care programs.

Key Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic condition management
• Support patients in executing personalized care plans and monitor their progress through regular check-ins
• Document patient interactions and collaborate with the clinical team to address care concerns and improve workflows

Required Qualifications

• An active compact multi-state LPN/LVN license in the state of residence is required
• At least 5 years of clinical experience as a Licensed Practical Nurse is required
• Remote work readiness, including stable high-speed WiFi, is required
• An active CA state LVN license is required
• Fluency in Spanish and experience in patient education for chronic conditions is preferred
