California Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (CA license required) to provide virtual patient support for Medicare patients managing chronic conditions.

Key Responsibilities

• Conduct outbound telephonic outreach to support patients in care programs
• Provide coaching and education to help patients manage chronic conditions and adhere to care plans
• Document patient interactions and collaborate with the clinical team to ensure comprehensive care

Required Qualifications

• An active compact multi-state LPN/LVN license in the state of residence is required
• At least 5 years of clinical experience as a Licensed Practical Nurse is required
• Remote work readiness, including stable high-speed WiFi, is required
• An active CA state LVN license is required
• Fluency in Spanish and experience with chronic condition management is preferred
