California Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (CA license required) to provide virtual patient support for individuals with chronic conditions.

Key Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic condition management
• Educate patients on disease management, medication adherence, and preventive care strategies
• Document patient interactions and collaborate with the clinical team to ensure patient support and care coordination

Required Qualifications

• Active compact multi-state LPN/LVN license in the state of residence
• At least 5 years of clinical experience as a Licensed Practical Nurse
• Remote work readiness, including stable high-speed WiFi
• Active CA state LVN license
• Fluency in Spanish and experience in patient education for chronic conditions preferred
