Job Summary

To support a growing nephrology practice, the fully remote California Licensed Practical Nurse will conduct monthly clinical check-ins, manage patient engagement, and ensure accurate clinical documentation while collaborating with care teams and providers.

Key responsibilities

• Perform monthly patient engagement and outreach, building strong relationships with patients and caregivers
• Conduct structured symptom assessments and evaluate changes in patient conditions using clinical judgment
• Document all patient interactions in the EHR and CCM platform, ensuring compliance with regulatory standards

Required qualifications

• Active LPN or LVN license
• Bilingual (Spanish/English)
• Minimum 1-2 years of clinical experience in ambulatory care, population health, or care coordination
• Strong clinical assessment and documentation skills
• Ability to work effectively in a remote environment with reliable internet access
