Required Qualifications

• Active, uncumbered Registered Nurse license in the state of operation
• Strong knowledge of Healthcare Effectiveness Data and Information Set (HEDIS) and HIV AIDS Bureau (HAB) standards
• Experience in chronic care management and transitions of care
• Ability to work collaboratively within a team and adapt in a dynamic environment
• Commitment to patient retention and engagement in healthcare
