Job Summary

A company is looking for a Registered Nurse - APCM with a California license required.

Key Responsibilities

• Proactively monitor patient vitals and clinical data to identify care gaps
• Develop and manage individualized care plans in alignment with physician recommendations
• Engage with patients for support, medication adherence, and virtual follow-up visits

Required Qualifications, Training, and Education

• Active California RN License and multi-state RN Compact State Licensure
• Associate Degree in Nursing Science
• 5+ years of recent clinical experience as an RN in an ICU/ER setting preferred
• Experience in chronic care management programs is a plus
• Technical fluency with multiple platforms and systems, including EMR experience
