Job Summary

A company is looking for a Registered Nurse (RN) - Enhanced Care Management (ECM).

Key Responsibilities

• Provide clinical oversight and guidance to Community Health Workers (CHWs) and Behavioral Health Care Managers (BHCMs)
• Lead clinical case reviews to ensure care plans are appropriate and aligned with health plan requirements
• Support care coordination across medical, behavioral health, and social needs

Required Qualifications

• Active Registered Nurse (RN) license in California
• 3+ years of clinical nursing experience
• Experience in care management, population health, or community-based care
• Bilingual (Spanish)
• Experience working with Medicaid or underserved populations
