Job Summary

Seeking a full-time California Licensed RN, the Hospital Care at Home RN will provide high-quality patient care remotely, utilizing critical thinking and assessment skills to support acute care patient populations in their homes.

Key responsibilities:

• Utilize monitoring and assessment skills to care for acute care patients in a home setting
• Conduct home and audio/visual patient care visits as directed by the care team
• Ensure efficient implementation of patient care plans with a patient-centered approach

Required qualifications:

• Active, unencumbered California state RN license
• Three years of RN experience, preferably in Med/Surg or inpatient settings
• Clinical knowledge to promote patient safety and positive experiences
• Ability to adapt to a dynamic work environment and collaborate effectively
• Commitment to ongoing learning and obtaining necessary certifications
