California Licensed RN Care Coordinator

Job Summary

To improve care coordination for patients with complex needs, the part-time California Licensed RN Care Coordinator will conduct assessments, develop individualized care plans, and provide education on disease self-management while working remotely within California.

Key responsibilities

• Coordinate care and collaborate with multidisciplinary teams to enhance clinical outcomes
• Conduct assessments to identify unmet needs and develop tailored care plans for patients
• Educate patients and their families on disease management and navigate the healthcare system effectively

Required qualifications

• Two years of clinical experience as an RN in relevant healthcare settings
• Current California Registered Nurse (RN) license
• Demonstrated leadership skills in guiding multidisciplinary teams
• Knowledge of regulatory standards and complex case management
• Ability to work independently and manage multiple tasks in a fast-paced environment
