California Licensed RN Care Navigator

Job Summary

To support kidney health management, the remote California Licensed RN Care Navigator will manage a caseload of members, conducting health screenings, creating care plans, and collaborating with healthcare teams to ensure effective patient outcomes.

Key responsibilities

• Handle in and outbound calls to provide exceptional service and education on kidney health and related conditions
• Engage members in HealthMap's Kidney Health Program and follow up based on policy guidelines
• Coordinate care as a liaison among patients, their support networks, and healthcare providers

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
