California Licensed RN Care Navigator

Job Summary

To facilitate high-quality kidney health management, the full-time California Licensed RN Care Navigator will manage a caseload of patients remotely, coordinating care, conducting health screenings, and collaborating with healthcare providers to ensure effective outcomes.

Key responsibilities

• Handle in and outbound calls to deliver exceptional service to members
• Educate patients on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program
• Collaborate with an interdisciplinary care team to coordinate care and resolve patient concerns

Required qualifications

• Active, unrestricted RN license in California
• Bachelor's degree or equivalent experience (6+ years of RN experience including 3+ years in case management may substitute for degree)
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
