California Licensed RN Care Navigator

Job Summary

To support kidney health management, the full-time California Licensed RN Care Navigator will manage a caseload of patients, facilitating care coordination and delivering education on renal health while working remotely within the PST and MST time zones.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service and support to members
• Educate patients on kidney health, dialysis options, and related co-morbid conditions to optimize care
• Collaborate with healthcare teams to coordinate care and resolve patient concerns effectively

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
