California Licensed RN Case Manager

Job Summary

A company is looking for a Registered Nurse RN Case Manager.

Key Responsibilities

• Coordinate and facilitate quality care for ESRD and CKD patients in collaboration with healthcare teams
• Manage individualized care plans and support patients through complex healthcare transitions
• Attend monthly meetings and meet with patients as needed based on business requirements

Required Qualifications, Training, and Education

• A.D.N degree from an accredited nursing school required; B.S.N preferred
• Current Registered Nurse (RN) license in the state of practice required
• Minimum of five (5) years' experience in clinical nursing required
• Minimum of three (3) years' experience in renal nursing preferred
• Case management experience is strongly preferred
