California Licensed RN Case Manager

Job Summary

A company is looking for a Remote RN Case Manager, SNP (Bilingual Preferred).

Key Responsibilities

• Coordinates care for members with complex and chronic care needs
• Completes comprehensive assessments and collaborates with the care team to implement care plans
• Ensures access to appropriate care and assists in resolving barriers to health improvement

Required Qualifications, Training, and Education

• Minimum 2 years of clinical experience and 1 year of case management experience
• Must have an active RN license in California (Non-Compact)
• Post High School courses completed to obtain RN licensure or an Associate's Degree in Nursing
• Preferred: Bachelor's degree in Nursing (BSN)
• Health Plan experience is preferred
