California Licensed RN Case Manager

Job Summary

Working remotely on a full-time basis, the California Licensed RN Case Manager will assess, plan, implement, and evaluate patient care while coordinating with clinicians and managing a caseload to optimize health outcomes.

Key responsibilities:

• Prioritizes patient care needs and addresses emerging issues during initial visits
• Consults with physicians and team members to implement and monitor care plans effectively
• Coordinates discharge provisions and follow-up care to ensure continuity of care for high-risk patients

Required qualifications:

• Graduation from an accredited school of nursing
• Active, unrestricted Registered Nurse license through the State of California and state of residence
• 1+ years of care management, utilization review, or discharge planning experience
