California Licensed RN Case Manager

Job Summary

To support members transitioning from hospital or skilled nursing care, the full-time Remote California Licensed RN Case Manager will develop personalized discharge care plans, coordinate services, and educate patients and families, focusing on reducing readmissions and ensuring continuity of care.

Key responsibilities

• Develop and manage personalized discharge care plans for patients
• Coordinate services across providers and community resources to support patient recovery
• Educate members and families on diagnoses, medications, and self-care practices

Required qualifications

• Active, unrestricted California RN license
• Bilingual in Spanish or Vietnamese
• 2+ years of case management or clinical experience
• Willingness to obtain licensure in other designated states within the first 6 months of employment
• Knowledge of Medicare Managed Care Plans and community resources
