Job Summary
To enhance care coordination, the full-time California Licensed Social Care Manager will work remotely to collaborate with patients, caregivers, and healthcare teams, conducting comprehensive psychosocial assessments and developing tailored action plans to address social determinants of health.
Key responsibilities:
- Utilize approved resources to create effective care management plans for members
- Conduct comprehensive psychosocial assessments and develop member-specific action plans
- Advocate for members and assist them in navigating the healthcare system
Required qualifications:
- Master's degree from a CSWE-accredited School of Social Work or a related Behavioral Science field
- 1+ years of clinical experience in a healthcare setting or completion of a relevant internship
- Working knowledge of healthcare delivery systems
- Ability to maintain confidentiality of patient information in accordance with HIPAA guidelines
- Experience in collaborating with interdisciplinary teams and community agencies
