California Licensed Social Worker

Job Summary

Providing integrated medical, behavioral, and social care to patients with complex healthcare needs, the full-time California Licensed Social Worker will conduct comprehensive assessments and deliver evidence-based therapies remotely while collaborating with an interdisciplinary care team.

Key responsibilities

• Support the behavioral health needs of patients through assessments and evidence-based treatment for mental health issues
• Collaborate with prescribing providers for psychotropic medication management and ensure treatment adherence
• Document patient encounters and facilitate effective communication regarding patients' behavioral health among clinical team members

Required qualifications

• Master's degree in Social Work
• Current California independent licensure required
• 2+ years of prior healthcare or behavioral health experience
• Geriatric care experience highly desired
• Proficient in Microsoft Office Suite and EMR documentation
