California Licensed Social Worker

Job Summary

Providing integrated medical, behavioral, and social care to patients with complex healthcare needs, the full-time California Licensed Social Worker will deliver clinical care through virtual and telephonic visits while collaborating with a multidisciplinary team in a fully remote environment.

Key responsibilities

• Support the behavioral health needs of an assigned patient caseload using evidence-based models of care
• Collaborate with prescribing providers on psychotropic medication management and monitor treatment adherence
• Document patient encounters and assessments in electronic health records, ensuring timely and accurate records

Required qualifications

• Master's degree in Social Work
• Current California independent licensure required
• 2+ years of prior healthcare or behavioral health experience
• Geriatric care experience highly desired
• Proficient in Microsoft Office Suite and electronic medical records (EMR) systems
