California Licensed Social Worker
Job Summary
Providing integrated medical, behavioral, and social care to patients with complex healthcare needs, the full-time California Licensed Social Worker will engage in virtual and telephonic visits, conduct comprehensive behavioral health assessments, and collaborate with interdisciplinary teams to develop treatment plans in a fully remote capacity.
Key responsibilities
- Support the behavioral health needs of an assigned patient caseload through evidence-based treatment for mental health issues
- Collaborate with prescribing providers on psychotropic medication management and monitor treatment adherence
- Document patient encounters and assessments in electronic health records, ensuring timely and accurate communication within the clinical team
Required qualifications
- Master's degree in Social Work
- Current California or Washington independent licensure required
- 2+ years of prior healthcare or behavioral health experience
- Geriatric care experience highly desired
- Proficient in Microsoft Office Suite and electronic medical records (EMR) systems
