California Licensed Specialty Nurse Navigator

Job Summary

To support patient care coordination, the full-time California Licensed Specialty Nurse Navigator will navigate patients through the continuum of care, establish nursing goals, and provide in-person clinic visits, with one remote shift per week and no weekend or holiday work required.

Key responsibilities

• Navigating patients through the continuum of care as a liaison between physicians, departments, and patients
• Establishing specific nursing goals to guide care planning and promote self-reliance
• Providing disease and treatment-specific education tailored to individual patient goals

Required qualifications

• BSN with 3 years of recent clinical nursing experience in various settings
• California RN License
• BLS certification if working in a clinical setting
• ONS Chemotherapy and Biotherapy certification
• Preferred: MSN and OCN certification for hematology or ONN-CG for oncology
