California Licensed Telehealth Nurse Practitioner

Job Summary

Covering weekend tele visits for home health patients in the SoCal/OC region, the part-time California Licensed Telehealth Nurse Practitioner will conduct remote assessments from 5 PM to 11 PM, manage Circadia platform alerts, and ensure continuity of care for high-risk patients.

Key responsibilities

• Conduct follow-up and urgent telehealth assessments on flagged patients and coordinate with on-call providers
• Interpret trend-based insights from the Circadia platform to prioritize high-risk patients and prevent unnecessary ED visits
• Complete same-day documentation using AI-powered tools for all tele visits

Required qualifications

• Certified and licensed Nurse Practitioner in California with ANCC (or equivalent) in Adult/Family/Geriatric/Acute
• Eligible for Medicare participation and able to practice independently
• 2+ years of NP experience with complex adult populations in home health or post-acute settings
• Comfortable conducting telehealth visits and navigating virtual care platforms
• Availability for after hours work from 5 PM to 11 PM daily
