California Licensed Telehealth RN

Job Summary

To support a growing telehealth initiative, the full-time California Licensed Telehealth RN will remotely monitor and manage patients with heart failure, utilizing clinical skills to assess vital signs, provide education, and coordinate care with the clinical team.

Key responsibilities

• Triage patients using protocols to determine appropriate nursing measures and escalation of care
• Conduct virtual nursing assessments to evaluate patient symptoms, medication adherence, and disease progression
• Provide education on heart failure management, including medications and potential side effects, to patients and caregivers

Required qualifications

• Bachelor's Degree in Nursing
• Minimum of 3 years of experience in cardiology, heart failure management, or critical care nursing
• Current license to practice as a Registered Professional Nurse in California
• Strong background in heart failure patient management and knowledge of Guideline-Directed Medical Therapy (GDMT)
• Fluency in English and Spanish is highly preferred
