Job Summary
To support a growing team dedicated to improving senior care, the full-time Bilingual Spanish Telephonic RN Case Manager will provide telephonic case management for medically complex and chronically ill members, conducting health assessments and coordinating care remotely while working Pacific Time hours.
Key responsibilities
- Provide telephonic case management to medically complex and chronically ill members
- Conduct comprehensive health assessments and create individualized care plans
- Coordinate care with internal and external partners, including physicians and specialists
Required qualifications
- Active, unrestricted RN license in California (Non-Compact)
- Minimum 2 years of clinical nursing experience
- At least 1 year of case management experience
- Must be bilingual in English and Spanish
- Proficiency with Microsoft Office (Word, Excel, Outlook)
