Job Summary

A company is looking for a Patient Enrollment Specialist.

Key Responsibilities

• Engage with prospective clients and document interactions in the CRM system, making 60-100 outbound calls daily
• Facilitate personalized care planning by conveying client preferences to the care team
• Assist clients in completing psychosocial assessments and developing actionable care plans

Required Qualifications

• Active California LVN license, Medical Assistant certificate, or Bachelor's degree in a related field with 2+ years of direct healthcare experience
• Experience working with vulnerable populations
• Comfortable with technology, including video conferencing and electronic health records
• Prior experience in a remote role with a secure workspace
• Bilingual (English/Spanish) preferred
