California Licensed Vocational Nurse
Job Summary
To support patient care coordination in a remote environment, the part-time California Licensed Vocational Nurse will assist with telehealth-enabled case management activities, ensuring timely and appropriate services under RN/provider supervision from 3 PM to 11:30 PM PST.
Key responsibilities
- Coordinate day-to-day care activities for a virtually managed patient population
- Support patient throughput and case management activities under RN/provider direction
- Integrate real-time workflow execution with clinical coordination aligned with care plans
Required qualifications
- Active LVN/LPN license in good standing, with a preference for California License
- Minimum of 2 years of clinical experience in acute, home health, ambulatory, or case management settings
- Experience with EMR/EHR systems and clinical workflows
- Strong telephonic and virtual communication skills
- Ability to manage multiple priorities in a fast-paced remote environment
