California Licensed Vocational Nurse
Job Summary
Supporting patient throughput and care coordination, the part-time California Licensed Vocational Nurse will function as a telehealth-enabled care coordinator within an Advanced Care at Home program, working remotely from 7 a.m. to 3:30 p.m. PST under RN/provider supervision.
Key responsibilities
- Coordinate day-to-day care activities across a virtually managed patient population
- Integrate real-time workflow execution with case management support to ensure timely and appropriate services
- Assist in clinical-adjacent coordination while adhering to LVN licensure scope and regulatory standards
Required qualifications
- Active LVN/LPN license in good standing; California license preferred
- 2+ years of clinical experience in acute care, home health, ambulatory, or case management
- Experience with EMR/EHR systems and clinical workflows
- Strong telephonic and virtual communication skills
- Ability to manage multiple priorities in a fast-paced remote environment
