California Licensed Vocational Nurse
Job Summary
To support the Enrollments team, the full-time California Licensed Vocational Nurse will engage with prospective clients through 60-100 outbound calls daily, educate them on Enhanced Care Management services, and facilitate their enrollment while working remotely.
Key responsibilities
- Engage with prospective clients and document interactions in the Client Relationship Management (CRM) system
- Assist clients in completing psychosocial assessments and developing personalized care plans
- Review medical charts to understand client needs and recommend appropriate interventions
Required qualifications
- Active California Licensed Vocational Nurse (LVN) license, Medical Assistant certificate, or Bachelor's degree in a related field
- Minimum of 2 years of telephonic outreach or enrollment experience in a high call volume environment
- Experience working with complex care clients and ECM Populations of Focus
- Familiarity with technology, including video conferencing and electronic health records
- Bilingual (English/Spanish) is highly preferred
