Job Summary
To support a growing healthcare practice, the full-time California Licensed Vocational Nurse Care Coordinator will manage a panel of 400-450 patients, conducting monthly outreach and health coaching while working remotely in a HIPAA-compliant home office.
Key responsibilities
- Conduct 50-60 daily outbound calls to provide care plan support and ensure continuity of care
- Collaborate with clinical teams and caregivers to address patients' clinical and social needs
- Document all patient interactions in real time, maintaining HIPAA compliance and accurate records
Required qualifications
- Active California Licensed Vocational Nurse (LVN) license
- Minimum of 2 years of clinical experience in a healthcare setting
- Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
- HIPAA-compliant home office setup with required internet capabilities
- Must be legally authorized to work in the United States
