Job Summary

Key Responsibilities

Assess member needs and develop client care plans
Coordinate health care benefits and facilitate access to care
Maintain patient health records and complete documentation in compliance with care plans

Required Qualifications

Active, unrestricted California Licensed Vocational Nurse (LVN) license
2+ years of experience as a care manager, care navigator, or community health worker
Working knowledge of government and community resources related to social determinants of health
Willingness to work Monday-Friday with potential for evenings and weekends
Bilingual (English/Spanish) is highly preferred
