Required Qualifications

Active, unrestricted Licensed Vocational Nurse (LVN) license in California
2+ years of experience as a care manager or similar role
Knowledge of government and community resources related to social determinants of health
Willingness to work Monday-Friday with possible evenings and weekends
Bilingual (English/Spanish) highly preferred
