Job Summary
To support patient care initiatives, the full-time California Licensed Vocational Nurse Coordinator will manage a panel of 400-450 patients, providing health coaching and conducting monthly outreach in a remote work environment.
Key responsibilities
- Conduct 50-60 daily outbound calls to patients, ensuring continuity of care and providing tailored health coaching
- Collaborate with clinical teams and caregivers to address patient needs and close gaps in care through various management services
- Document all patient interactions in real time while maintaining HIPAA compliance and supporting care coordination goals
Required qualifications
- Active California Licensed Vocational Nurse license or multi-state compact license
- Minimum of 2 years of clinical experience in a healthcare setting
- Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
- HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability
- Must be legally authorized to work in the United States
