California Patient Care Coordinator

Job Summary

A company is looking for a Patient Care Coordinator to manage patient interactions and scheduling for remote monitoring services.

Key Responsibilities

• Make outbound calls to newly-referred patients, explaining the program and securing consent
• Schedule initial in-home visits and manage appointment confirmations and reschedules
• Maintain accurate records and provide feedback to improve patient experience

Required Qualifications

• 5+ years of experience in patient-facing healthcare operations or similar roles
• Bilingual in English and Spanish, fluent in both written and spoken communication
• California residency and ability to work standard Pacific Time business hours
• Strong knowledge of HIPAA and patient privacy practices
• Experience with high outbound call volumes and working with elderly patients or those with chronic conditions
