California RN Care Navigator

Job Summary

A company is looking for an RN Care Navigator (100% Remote for Pacific & Mountain time zones).

Key Responsibilities

• Manage in and outbound calls to provide exceptional service to members
• Educate members on kidney health, treatment options, and engage them in the Kidney Health Program
• Coordinate care among patients, their support networks, and healthcare providers to ensure positive outcomes

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
