California RN Care Navigator

Job Summary

Managing a caseload of kidney health patients, the full-time RN Care Navigator/Case Manager will facilitate high-quality care coordination and outcomes while working remotely in the PST and MST time zones.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service and education on kidney health and treatment options
• Collaborate with healthcare providers and community resources to create and maintain member-centric care plans
• Advocate for patients by addressing concerns and barriers to ensure effective care delivery and self-management

Required qualifications

• Active, unrestricted RN license
• Bachelor's degree or equivalent experience (6+ years of RN experience, including 3+ years in case management)
• CCM certification preferred
• Three years of case management experience preferred
• Experience in a dialysis or transplant center preferred
