Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities

Conduct clinical assessments and develop individualized ECM care plans
Provide RN clinical oversight and collaborate with care teams to monitor member outcomes
Ensure compliance with Medi-Cal, CalAIM, and ECM program requirements through documentation and audits

Required Qualifications

Active California RN license
Associate Degree in Nursing (ADN) required; BSN preferred
Minimum 3 years of clinical nursing experience
Experience working with Medi-Cal and/or managed care populations
Ability to work independently and collaboratively
