California RN Case Manager

Job Summary

Responsible for health care management and coordination, the fully remote California RN Case Manager will manage care for members with complex and chronic needs, utilizing the nursing process and engaging with members and interdisciplinary teams via telephone.

Key responsibilities

• Coordinates care by serving as a resource for members, their families, and physicians
• Completes comprehensive assessments to evaluate members' health status and service needs
• Collaborates with care teams to implement and manage individualized care plans

Required qualifications

• Active, valid, and unrestricted RN license in California (Non-Compact)
• Minimum 2 years of clinical experience and 1 year of case management experience
• Post High School courses completed to obtain RN licensure or an Associate's Degree in Nursing
• High level of understanding of community resources and management of chronic conditions
• Proficiency in PC-based software programs, including Word, Excel, and PowerPoint
