Job Summary
A company is looking for a Telecare Care Coach.
Key Responsibilities
- Manage patient demographics, schedule appointments, and maintain program information
- Coordinate meetings and record minutes for the Interdisciplinary Care Team
- Respond to inquiries from patients, providers, and internal departments while performing outreach calls
Required Qualifications
- High School diploma required
- Three years of customer service or transaction processing experience in a Managed Care setting
- Knowledge of Medicare Advantage and enrollment activities preferred
- Proficiency in MS Office applications including Word, Excel, and Access
- Ability to work independently and manage a high workload in a fast-paced environment
