Job Summary

A company is looking for a Telecare Care Coach.

Key Responsibilities
  • Manage patient demographics, schedule appointments, and maintain program information
  • Coordinate meetings and record minutes for the Interdisciplinary Care Team
  • Respond to inquiries from patients, providers, and internal departments while performing outreach calls
Required Qualifications
  • High School diploma required
  • Three years of customer service or transaction processing experience in a Managed Care setting
  • Knowledge of Medicare Advantage and enrollment activities preferred
  • Proficiency in MS Office applications including Word, Excel, and Access
  • Ability to work independently and manage a high workload in a fast-paced environment
