Job Summary

Empowering members on their health journeys, the full-time Cardiovascular Clinical Guide will conduct comprehensive hypertension assessments, provide lifestyle counseling, and coordinate multidisciplinary care in a remote setting.

Key responsibilities

• Screen members for eligibility in the Hypertension Clinic and conduct comprehensive assessments
• Deliver intensive lifestyle and nutrition counseling to promote heart-healthy behaviors
• Coordinate care with multidisciplinary teams and serve as the primary point of contact for members

Required qualifications

• Experience as a Medical Assistant or in a clinical/healthcare setting
• At least 2 years of work experience in longitudinal chronic disease management
• Strong coaching skills to support members in achieving health goals
• Ability to conduct motivational interviewing to influence positive behavior changes
• Experience with hypertension or cardiovascular health is preferred
